TO MAKE AN APPOINTMENT

Making an appointment is easy! Just follow these steps:

Call your insurance company to see if they require a doctor's prescription or a doctor's referral. Make sure you are aware of your co-pays and deductibles. Ask what diagnoses your plan will cover for speech services. Note: Do not assume that your insurance will cover speech services. Speech Center will hold you responsible for any costs your insurance plan does not cover!
Obtain any necessary referrals or authorizations your insurance requires from your primary doctor. Fax 520-829-7743
Call Speech Center at 520-721-4544 to schedule an appointment.
Bring in any documents that pertain to your child's development, health or speech to the appointment. We require completed Registration and Child History forms. You will find all the forms on the REQUIRED FORMS tab above. Bring your insurance card!

Park in front of the 1760 building, just off River near the West end of the Cambric Center. Enter under a brown canopy marked "Cambric" and go down the stairs to your right. Follow the hallway and take a right into an inside hallway, we are on your right.
